  • Yes, e-QIP will accept APO and FPO addresses. The APO/FPO should be entered in the city field and the AE, AA or AP should be entered in the state. (These are available at the bottom of the drop-down menu). Lastly, enter the zip code for the APO/FPO location.

  • e-QIP will requires all users to register by clicking the "Register for Username and Password" button on the right-hand side of the screen, inputting your Social Security Number (SSN) or request ID if you do not have a SSN, answering default Golden Questions, creating a username and password and then creating Challenge Questions. Username and password will be all that is needed to login after initial registration. Challenge Questions can be used to reset your password. If you forget your username you will have to contact your sponsoring agency for help. You may also need to contact your sponsoring agency to verify that you currently have an active request in the e-QIP system. The e-QIP system is used by a wide variety of Federal contractor and military personnel, and it is important that each user contact the appropriate personnel for assistance in completing their questionnaire. Please pay close attention to the following options:
    • Federal, military, Department of Defense (DoD) employees or applicants, and non-DoD Federal contractors: Initial point of contact for Golden Questions resets, forgotten usernames and other technical support should be the local security or human resource official of your sponsoring organization/agency.Your sponsoring agency is typically the individual or office that emailed you regarding initiating your eQIP form.  They are able to provide your user name and reset your account if needed.  If your agency resets your account, you will need to register again.
      • NOTE for Military and DoD local security or human resource officials: The DoD Security Services (Call) Center is not able to view e-QIP or reset the Golden Questions for applicants whose e-QIP questionnaires are not initiated/managed within the Joint Personnel Adjudication System (JPAS).
    • Industry (DoD Contractor) employees or applicants: Initial point of contact for Golden Question resets and other technical support is the DoD Security Services (Call) Center at 1-888-282-7682.

  • The first time you login to the e-QIP system after June 24, 2012, you will need to register. You register by clicking the "Register for Username and Password" button on the right-hand side of the screen. You will need to input your Social Security Number (SSN) or request ID if you do not have a SSN, answer your default Golden Questions and then you will create three Challenge Questions. It will then prompt you to create a new username and password. If you have already registered and do not remember your username or the answer to your Challenge Questions, you must contact your sponsoring agency.   Your sponsoring agency is typically the individual or office that emailed you regarding initiating your eQIP form.  They are able to provide your user name and reset your account if needed.  If your agency resets your account, you will need to register again.  General information regarding eQIP and frequently asked questions (FAQs) can be found at:  http://www.opm.gov/investigations/e-qip-application/#url=Need-Help . For more specific information you will need to seek the guidance of your sponsoring agency.  Note: You can only log into e-QIP when your sponsoring agency grants you access.

  • Yes, you can use your personal computer - at home or at the office. e-QIP is designed to be accessible from most computers, anywhere in the world. A browser configuration check will alert you to any incompatibilities with browsers or browser versions before you login to the e-QIP application.

  • In e-QIP, your sponsoring agency has the ability to grant you access to your Standard Forms previously filled out in e-QIP. Once your agency grants you access to your previous forms, you have 20 days to login and retrieve them. You will login with the same username and password you used to fill out your forms in e-QIP. If it is the first time accessing e-QIP since June 24, 2012 you will need to register and create a username and password first. Register by clicking the "Register for Username and Password" button on the right-hand side of the screen, inputting your Social Security Number (SSN) or request ID if you do not have a SSN, answering default Golden Questions, creating a username and password and creating Challenge Questions. Username and password will be all that is needed to login after initial registration. Alternatively, if you are seeking an electronic copy of a previously completed standard form application that you did not fill out in e-QIP, or an agency already  gave you access to your forms in e-QIP but you need additional information, then you should submit a request to the FIS Freedom of Information and Privacy Act (FOI/PA) office.   Details about submitting a request to FIS FOI/PA are available at: http://www.opm.gov/investigations/requesting-investigation-copies/federal-applicants-employees-or-contractors/.

  • If you have not certified your responses, please use the navigation bar in the top right corner to navigate to the sections and questions you need to change. You will be able to change your responses and provide additional information, if needed. If you have certified your responses, you cannot make any changes. Please release the form to your agency. Your sponsoring agency has the ability to return the request back to you in a manner that allows you to make amendments to your form. Please contact your sponsoring agency if you would like the form to be returned to you. Note: Each time the form is returned to you from your sponsoring agency in e-QIP, you will need to answer all "Yes/No" questions again and resign the certification and release forms.
