  2. Do I list the information as of December 31 or as of the date when I am completing the FDF? Specifically, my marital status changed after December 31 and I'm not sure what to report.

  1. What is meant by the suffix question?

    The term suffix refers to situations whereby names are passed from generation to generation. Examples include: junior; senior; second.

  2. Do I list the information as of December 31 or as of the date when I am completing the FDF? Specifically, my marital status changed after December 31 and I'm not sure what to report.

    All information provided on the FDF is as of December 31. Therefore, you would answer with your "old" marital status. The reporting period is the preceding calendar year, January 1 through December 31.

  3. I was legally separated as of December 31. Am I required to furnish my spouse's financial information?

    No. Please provide an explanation in the Comments field that you are married but legally separated. Please select "Legally Separated" as your filing status.

  4. I just got married. Should I be answering these questions individually or jointly with my spouse in mind, since he is also employed by the same Agency?

    If you were married as of December 31, choose "married" as your marital status.
