Federal Investigations Notice

OPM has revised the Standard Form 87/87A (SF 87/87A), Fingerprint Chart.  The revision updates the Privacy Act Statement.  The Fingerprint Charts are used in processing fingerprint checks submitted to the Federal Bureau of Investigation (FBI) to assist in determining whether an applicant is suitable for Federal employment and/or should be granted a security clearance.

The new revision date for the SF 87 (National Stock No. 754000 634 4037) and SF 87A (National Stock No. 754001 269 3384) is April 2006.  The GSA supply of previous versions of the SF 87 and SF 87A has been depleted.  However, we have made provisions to accept either form (revisions February 2002 and April 2006) with requests for investigation for the foreseeable future.
