Panama Canal Area Benefit Plan Changes for 2004


This is a general description of the plan changes. This page is not an official statement of benefits. For that, go to the Benefits descriptions in the Plan Brochure. Also, we edited and clarified language throughout the brochure; any language change not shown here is a clarification that does not change benefits.

  • Your share of the premium will increase by 5% for both Self Only and Self and Family.

  • We added a benefit for routine osteoporosis screening for all women 65 and older and routine osteoporosis screening, beginning at age 60 for women who are at increased risk. See page 22.

  • We revised the benefit for influenza vaccines to expand coverage for one annual influenza vaccine to all plan members age 50 and over. See page 23.

  • We added a benefit for a colonoscopy and double contrast barium enema, once every five years, for all plan members beginning at age 50. See page 22.

  • We added a benefit to provide all diabetics with specific prescription drugs and supplies, included on the Plan's formulary, to treat diabetes and its complications. See page 27.

  • We clarified the vision services benefit to specifically identify the conditions under which we cover eyeglasses. See page 28.

  • We clarified the hearing exam benefit to specifically describe what is included in the annual audiologist visit. See page 28.

  • We clarified the prescription drug benefit to specifically state that only FDA approved prescribed drugs, or their equivalents, are covered under this plan. See page 46.

  • We clarified that the Plan must be notified of all emergency admissions within two business days of the admission even if the patient has been discharged. See page 41.
