• A new option, Open Access Deductible, has been added.

• MRI / CT scans - 80% coverage from 100% coverage previously. Member responsibility is 20% of charges.

• Outpatient hospital visit - you will pay a $15 copayment for services provided or renewed during an outpatient hospital visit.

• The number of days of confinement in a skilled nursing facility is limited to 120 days when necessary or appropriate. Previously the limit was 180 days.

• Disposable needles and syringes for the administration of covered medications is covered under Durable Medical Equipment.
