HMO Health Ohio Changes for 2004


This is a general description of the plan changes. This page is not an official statement of benefits. For that, go to the Benefits descriptions in the Plan Brochure. Also, we edited and clarified language throughout the brochure; any language change not shown here is a clarification that does not change benefits.

  • Your share of the non-Postal premium will increase by 13% for Self Only or 18.6% for Self and Family.

  • Retail prescription drug copayment for non-formulary brand name drugs is now $30. Copayment for generics and formulary brand name drugs have not changed.

  • Mail order prescription drug copayment for non-formulary brand name drugs is now $60. Copayments for generics and formulary brand name drugs have not changed.

  • Members who purchase a brand name drug when a generic drug is available now pay the GENERIC copayment in addition to the difference between the cost of the generic drug and brand name drug. Previously, member paid the BRAND NAME copayment in addition to the difference between the cost of the generic drug and brand name drug.
