M感LAN Patient Safety Initiatives


MPlan has a Patient Safety Committee with a chairperson who oversees it and does not have a physician or hospital quality service or recognition program at this time.

Medicine Error Prevention Projects at M-Plan

M-Plan has a variety of patient safety projects underway to prevent medicine errors. These projects include the following -

  • ICARES
  • Polypharmacy
  • Drug Utilization Review
  • Dose Optimization
  • Ask a Pharmacist
  • Mail Order

ICARES

This is a doctor and patient education project to make the community more aware that taking unnecessary antibiotics can cause more harm than good. By taking antibiotics for a simple cold or flu (illnesses caused by a virus, NOT a bacteria) a patient can develop a resistance to the antibiotic and when they are really ill, it not work.

Polypharmacy

This is a project to make both the patient and their doctor aware of drug - drug interactions, duplicate treatments and ways to combine treatments to decrease patient copays. Patients who fill more than 10 medicines each month are identified and asked to come meet with their pharmacist to discuss the above safety issues. Doctors are sent a list of all the patient's medicines to see if they can safely and effectively change and/or decrease any medicines.

Drug Utilization Review

M-Plan provides on-line services to 100% of our pharmacies with up-to-date information on a patient's medicine history and important facts about medicines.

Dose Optimization

M-Plan reviews the medicine profiles of its members to determine if the dose of medicine they are taking in 2 or more pills can be given in 1 larger dose pill. This means a lesser copay for patients and encourages compliance with doctor's orders.

Ask a Pharmacist

All M-Plan patients receive a wallet-size card of important questions that should be asked each time a patient fills a prescription. This allows the patient to make better healthcare decision because they understand the treatment their doctor has ordered.

Mail Order

This project allows members to receive their medicines in the convenience of their own home. The mail order medicine company keeps track of the member's medicines and when they need refilled. Medicines are delivered with a three-month supply that keeps members from running out of their medicine and allow them to remain compliant with their doctor's orders.

We also include patient safety information in physician newsletters. Recent articles have included: appropriate antibiotic use and The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) patient safety goals.
