ConnectiCare Patient Safety Initiatives


ConnectiCare's patient safety initiatives are coordinated by our Quality Improvement Committee, and cross functional team with representatives from Health Services, Pharmacy Management, Disease Management, Credentialing, and Customer Services. The initiatives they oversee help increase member safety through education materials distributed through the mail and messaging at the point of service. Programs provide a safety net for important drug and member specific pharmaceutical care concerns. Following is a high level overview of some of our key initiatives.   • ConnectiCare has five health management programs that address the following conditions:
    • Asthma
    • Diabetes
    • Heart Failure
    • Coronary Artery Disease
    • High Risk Pregnancy
  • Members are identified for the programs through a review of the claims systems or the pregnancy precertification process.
  • Identified members are targeted according to their level of risk.
  • Higher risk members are targeted for outreach.
    • These members are contacted by the program and offered individual case management from a registered nurse.
    • Members enrolled case management receive periodic calls from a specialty RN to educate them about their condition, treatment options and medications.
    • The frequency of case management contact is determined by the RN's assessment of the member's need for education.
    • As necessary the RN may contact and discuss a member's condition with that member's treating physician.
  • In addition to case management, all health management programs do periodic educational mailings to members who are identified with a given condition.
    • These mailings usually promote awareness of a specific topic or aspect of the condition that is recognized as being important to reinforce.
  • Physicians whose members are enrolled in a health management program receive reports.
  • ConnectiCare uses the following recognized sources that are approved by board certified physician advisors for its evidenced-based clinical practice guidelines:
    • Asthma: National Heart, Lung, and Blood Institute (NHLBI)
    • Coronary Artery Disease: National Cholesterol Education Expert Panel on Detection, Evaluation and Treatment of High Blood Cholesterol in Adults (NCEP/ATP III). Joint National Commission on Prevention, Detection, Evaluation and Treatment of High Blood Pressure (JNC VI)
    • Heart Failure: American Heart Association/ American College of Cardiology (AHA/ACC)
    • Diabetes: American Diabetes Association (ADA)
    • High-Risk Pregnancy: Institute for Clinical Systems Improvement.
