Vista Healthplan Patient Safety Initiatives

VISTA takes the responsibility of patient safety very serious. We enlist all contact points to assist in monitoring how our business contributes to the health, wellbeing, privacy and safety of our members. Our medical operations, Quality Management and Pharmacy departments have all taken up the banner of accountability to assure the welfare of members. This arrangement allows VISTA to capture data from all service vectors of the healthcare delivery system (physician, hospital and pharmacy).

We also review claims for the type of care being received and or /recommendations to determine if it is appropriate for the diagnosed condition. By monitoring the claims received, we are able to apply quality management and care initiatives to a patient's plan of treatment.

As a result of our monitoring all vectors of healthcare to assure that our members are receiving appropriate treatment and care, we have developed several disease management and case management programs. They include: Congestive Heart Failure, Diabetes, Asthma, Coronary Heart Disease and End Stage Renal Disease, Pain Management programs and Obstetrics case management program.

We continue to monitor high-risk pregnancies. Once identified, women in this category are monitored in the areas of prenatal care and any special needs. We assist them through education and interaction throughout their pregnancy.

In addition, we practice our commitment by monitoring care from admission to discharge and follow up care after discharge. Using this method helps to avoid duplication of services and duplication of medication.

On the mental health front, we are using many of the same safety protocols to intervene and improve the health and welfare of our members. We monitor the use of psycho-pharmacological medications to assure appropriate use and reduce adverse drug interactions. Also, we have partnered with a mental health provider to follow the member through admission and discharge. We encourage early follow up after discharge from psychiatric facilities to assure continuity and coordination of care.

At Vista, we understand the role that consumer education plays in health and wellness and for this reason we have developed a state of the art work-site wellness program. Our members can improve their quality of life by participating in our health fairs, screening, lectures and much more.
