Health Alliance HMO Changes for 2004


This is a general description of the plan changes. This page is not an official statement of benefits. For that, go to the Benefits descriptions in the Plan Brochure. Also, we edited and clarified language throughout the brochure; any language change not shown here is a clarification that does not change benefits.

  • Your share of the non-Postal premium will increase by 14.4% for Self Only or 14.4% for Self and Family.

  • The counties of Dallas, Jasper, and Polk have been added to the Ames Iowa Service Area.

  • We have added the Peoria Service Area, which consists of the Illinois counties of Peoria, Tazewell, and Woodford.

  • Macon county is no longer part of the Springfield Service Area. Macon county is now in the Decatur St. Mary's Service Area.

  • We have added the Sterling/Rock Falls Service Area, which consists of the Illinois counties of Bureau, Carroll, Lee, Ogle, and Whiteside.

  • The Plan's I Can Quit smoking cessation program has been added to the educational classes and programs benefit.

  • Inpatient rehabilitation services will be changed to a combination of 120 days per calendar year for skilled nursing care and rehabilitative therapy in an approved nursing or rehabilitative facility.

  • A mail order pharmacy program has been added to the prescription drug benefit.
