• Your share of the non-Postal premium will increase by 6.7% for Self Only and decrease by 0.1% for Self and Family.

• We have no benefit changes.

• We have expanded our Service Area to include the parishes of Claiborne and Natchitoches in the Shreveport/Alexandria areas. See page 7.

We have eliminated Enrollment Code AQ for the 2004 contract year. If you are currently enrolled in Code AQ and you do not make a change at Open Season, you will be transferred automatically to the Plan's remaining Code MV.
