AmCare 2V, ZG Terminating from the FEHB program effective November 1, 2002.
HMO Blue Texas Gulf Coast YM Service Area Reduction - in Central Texas of the following counties: Arkansas, Atascosa, Bastrop, Bee, Bell, Bexar, Blanco, Bosque, Brazos, Brooks, Burleson, Burnet, Caldwell, Cameron, Comal, Fayette, Freestone, Gonzales, Guadalupe, Hays, Hidalgo, Hill, Jim Wells, Karnes, Kendall, Kenedy, Kleberg, Lee, Leon, Madison, McLennan, Medina, Milam, Navarro, Nueces, Refugio, Robertson, San Patricio, Starr, Travis, Willacy, Williamson, Wilson
Note: Service will continue to be provided under enrollment code YM1 and YM2 in the Houston area. This will include the following counties: Austin, Brazoria, Chambers, Colorado, Fort Bend, Galveston, Grimes, Harris, Liberty, Matagorda, Montgomery, San Jacinto, Walker, Waller, Washington, and Wharton.
HMO Blue Texas Southwest YX Termination of code will include counties within the state of Texas in the following territories: Dallas/Ft. Worth and East Texas - Northeast Territory, West Texas, Lubbock and Amarillo - West Territory, and Central Texas - Central Territory.
Humana Health Plan of Texas UR Added new Standard Option
