Person receiving compensation payments from the Department of Labor (or his/her family member)

Choose one. The person receiving compensation payments:

  1. has received the payments for less than 12 months AND is still on an agency's rolls as an employee

  2. OR

  3. is NOT still on the agency's rolls as an employee AND/OR has been receiving the compensation payments for 12 months or more

If you aren't sure, contact the individual's agency or former agency.
