Subject: Federal Employees' Health Benefits (FEHB) Program: Special Open Season for Enrolles of Premier Health Care of Arizona, Enrollment Code 9A and 9B


Premier Health Care of Arizona, enrollment codes 9A and 9B, is insolvent and has ceased operations in the state of Arizona effective November 15, 1999. This plan will no longer participate in the FEHB Program.

Since the 2000 FEHB Guides are already printed, the plan's name appears in the Guides. Please do not allow employees to enroll in this plan.

We are authorizing a special open season for plan enrollees to choose new plans. If you have employees in the area served by the plan, we ask that you notify them directly of the plan's withdrawal.

This special open season is effective immediately and will continue through December 18, 1999. Agencies are, however, asked to apply this cutoff date liberally. Enrollment changes during this special open season may be made effective retroactive or prospective to the first day of a pay period that begins after November 15, 1999 and no later than December 18, 1999.

Since this special open season overlaps the regular open season, agencies will need to identify their employees enrolled in this plan, to ensure these changes are not processed as a regular open season change resulting in a year 2000 effective date.
