This is to advise you that Aetna/US Healthcare, Enrollment Code RD, has lost a significant number of its primary care provides in the Dayton, Ohio, area and will lose even more providers as of September 1, 1999.

We are therefore authorizing a special open season for Aetna/US Healthcare enrollees in the Dayton area during which they may change to another health plan.

Enrollees who do not change plans will remain enrolled in Aetna/US Healthcare and will have to obtain care from remaining plan providers.

Aetna/US Healthcare will send enrollees in the Dayton area a letter letting them know about this special open season.

PLEASE NOTE: This special open season applies only to Aetna/US Healthcare enrollees in the Dayton, Ohio, area.
