Subject: Federal Employees' Health Benefits (FEHB) Program: Special Open Season for Enrollees of Humana Health Plan, Inc., Enrollment Code TL

This is to advise you that Humana Health Plan, Inc., enrollment code TL, in the state of Nevada, has been sold and will no longer participate in the FEHB Program.

We are authorizing a special open season effective immediately for enrollees of the plan during which they may change to another plan.

Enrollees will have until March 24, 2000 to submit a Standard Form (SF) 2809 to change plans. Enrollment changes made during this special open season, should be made effective the first day of the first full pay period in March (March 12 for most employees). Enrollees who do not change plans during this special open season, will have no benefits for the remainder of the 2000 contract year.

The Plan will send enrollees a letter to advise them of this special open season. We have attached a copy of that letter for your information.
