Federal Employees Health Benefits (FEHB) Program: Opportunity for Enrollees of The Wellness Plan, Enrollment Code K3, to Change Enrollment |
The Wellness Plan (enrollment codes K31 and K32) in the State of Michigan, recently notified us of a serious problem with their provider network. The Wellness Plan will no longer contract with the Henry Ford Medical Center and the following St. John's Medical System locations:
- St John Hospital & Medical Center
22101 Moross Detroit, MI |
- St. John Macomb Hospital
11800 12 Mile Road Warren, MI

- Providence Hospital & Medical Center
16001 W. Nine Mile Road Southfield, MI

We are authorizing The Wellness Plan enrollees who are affected by the network disruption to select a new health plan. The opportunity to change enrollment will begin immediately and continue through May 18, 2002. Agencies may apply the cutoff date liberally as necessary. The effective date of all enrollment changes will be the first day of the pay period in which the enrollee makes the change.
The Plan will send the attached notice to all enrollees. If enrollees are uncertain whether or not they are affected by the network disruption, they should contact The Wellness Plan for assistance at (800) 875-9355.
