The Online 1417 CFC Results form is now available to report on 2004 campaign results.
Please complete the form no later than Sunday, March 6, 2005.
How To Access The Form:
The form is located at:
http://www.opm.gov/aboutus/combinedfederalcampaign/localcampaign/index.aspx#Campaign Info System. PCFOs have been sent an email with a PIN and instructions to gain access to the new 1417 Campaign Information System (CIS). Every campaign must create its own PIN, hint question, and hint response the first time they enter the 1417 CIS this year. Thereafter, the new PIN will be used by campaigns to complete 1417 CFC Results forms and to update LFCC and PCFO contact information.
In order to enhance the quality of the information for use by campaigns for benchmarking purposes, the following additional data is being requested:
- Breakout of local federation names and designations;
- Number of employees in military installations, postal facilities, and other Federal agencies in your campaign area;
- Number of contributors from military installations, postal facilities, and other Federal agencies in your campaign area;
- Number of Loaned Executives in 2004;
- Amount of funds collected by Federal agencies in your campaign area for the special solicitation for victims of the Asian tsunami.
