    Grand Valley Health Plan Patient Safety Initiatives

    1a.

    Grand Valley Health Plan adopted 20 of the AHRQ patient safety initiatives.

    b.

    Referrals for transplants or complex cases are put through a specialty review process.

    c.

    Grand Valley Health Plan attends the state-wide Patient Safety Work Group.

    d.

    Grand Valley Health Plan monitors and evaluates all concurrent and retrospective patient readmissions, deaths and quality of care cases for potential safety issues.

    e.

    Grand Valley Health Plan is in process of developing and implementing a patient safety web page on our external Internet site. In addition, we are sending a "Request for Information" to our network hospitals to use for hospital profiles that will also be on our external Internet site. This information will include the specific activities that each hospital is doing to improve patient safety.

    2a.

    By adopting these initiatives, Grand Valley Health Plan will meet and exceed benchmarks determined by the Michigan Health & Safety Coalition.

    b.

    This process includes verification of outcomes, experience in performing procedures, efficacy, and morbidity/morality rates. After this analysis, the member is referred to the Center of Excellence that can provide the member with the highest quality of care.

    c.

    This group provides a collaborative effort for patient safety initiatives in Michigan.

    d.

    By monitoring these readmissions, Grand Valley Health Plan can put processes in place to avoid future readmissions.

    e.

    This site will include tips on how a member can assist in making their care safer. There will be a link to the Michigan Health & Safety Coalition outcomes on our in-network hospitals. This will assist the member in making informed decisions about their healthcare.

    3a.

    On an annual basis, we do a year-end wrap up to compare our outcomes with the benchmarks established.

    b.

    Each case is managed by a Case Manager and a database is maintained where the data is pulled to do an annual evaluation.

    c.

    We review minutes and actions performed by the Work Group and include this performance in an annual evaluation.

    d.

    A database is maintained and an evaluation is performed on an annual basis. We also send surveys to patients that were in the hospital for an inpatient admission. This survey includes a question regarding their perception of the quality of care they received at the hospital. Members can call Grand Valley Health Plan at any time to file a complaint. A monthly report is generated and any patient safety issues are addressed.

    e.

    We will monitor the number of times this web page is visited on a quarterly basis.
