Beginning June 24, 2012, e-QIP will require all users to register by answering default Golden Questions, creating a username and password and creating Challenge Questions. Username and password will be all that is needed to login after initial registration. Challenge Questions can be used to reset your password. If you forget your username you will have to contact your sponsoring agency for help.
You may also need to contact your sponsoring agency to verify that you currently have an active request in the e-QIP system.
The e-QIP system is used by a wide variety of Federal contractor and military personnel, and it is important that each user contact the appropriate personnel for assistance in completing their questionnaire. Please pay close attention to the following options:
-
Federal, military, Department of Defense (DoD) employees or applicants, and non-DoD Federal contractors: Initial point of contact for Golden Questions resets and other technical support should be the local security or human resource official of your sponsoring organization/agency.
-
NOTE for Military and DoD local security or human resource officials: The DoD Security Services (Call) Center (1-888-282-7682) is not able to view e-QIP or reset the Golden Questions for applicants whose e-QIP questionnaires are not initiated/managed within the Joint Personnel Adjudication System (JPAS).
-
Industry (DoD Contractor) employees or applicants: Initial point of contact for Golden Question resets and other technical support is the DoD Security Services (Call) Center at 1-888-282-7682.

Dear OPM FAQ Staff: Question 25b on the SF-86 states, “To your knowledge, have you EVER had a clearance or access authorization denied, suspended, or revoked; or been debarred from government employment? If "Yes," give the action(s), date(s) of action(s), agency(ies), and circumstances.” My question is the following: If a person is notified by the FBI that they “did not successfully complete the preliminary processing stage in order to initiate a background investigation” and that “you may not be suitable for employment with the FBI”; based solely on that one letter, does that mean the person has officially (i.e. according to the SF-86, Q25b) “been debarred from government employment” as stated/meant in Q25b of the SF-86? Respectfully,

Questions about an agency’s action or whether that action is required to be reported on an investigative form should be referred to the agency that took the action.
In the context of the question on the Standard Form (SF) 86, a debarment refers to a suitability action that denies an individual examination for, or appointment to, a covered position. A covered position, as defined in title 5, Code of Federal Regulations, part 731 is one in the competitive service or one in the excepted service where the incumbent can be non-competitively converted to the competitive service, or a career appointment to a position in the Senior Executive Service.

The time needed to complete the application process depends on a number of factors, including whether a background investigation is needed prior to appointment and the type of investigation to be conducted. For national security positions, OPM is completing 90 percent of the initial security clearance investigations within an average of 40 days.
To determine the status of your investigation, you may call 724-794-5612, extension 7000. For information on the status of your application, please contact the office where you applied.
