Frequently Asked Questions Investigations

  • In e-QIP, your sponsoring agency has the ability to grant you access to your Standard Forms previously filled out in e-QIP. Once your agency grants you access to your previous forms, you have 20 days to login and retrieve them. You will login with the same username and password you used to fill out your forms in e-QIP. If it is the first time accessing e-QIP since June 24, 2012 you will need to register and create a username and password first. Register by clicking the "Register for Username and Password" button on the right-hand side of the screen, inputting your Social Security Number (SSN) or request ID if you do not have a SSN, answering default Golden Questions, creating a username and password and creating Challenge Questions. Username and password will be all that is needed to login after initial registration. Alternatively, if you are seeking an electronic copy of a previously completed standard form application that you did not fill out in e-QIP, or an agency already  gave you access to your forms in e-QIP but you need additional information, then you should submit a request to the FIS Freedom of Information and Privacy Act (FOI/PA) office.   Details about submitting a request to FIS FOI/PA are available at: http://www.opm.gov/investigations/requesting-investigation-copies/federal-applicants-employees-or-contractors/.

  • If you have not certified your responses, please use the navigation bar in the top right corner to navigate to the sections and questions you need to change. You will be able to change your responses and provide additional information, if needed. If you have certified your responses, you cannot make any changes. Please release the form to your agency. Your sponsoring agency has the ability to return the request back to you in a manner that allows you to make amendments to your form. Please contact your sponsoring agency if you would like the form to be returned to you. Note: Each time the form is returned to you from your sponsoring agency in e-QIP, you will need to answer all "Yes/No" questions again and resign the certification and release forms.

  • Yes, you can use your personal computer - at home or at the office. e-QIP is designed to be accessible from most computers, anywhere in the world. A browser configuration check will alert you to any incompatibilities with browsers or browser versions before you login to the e-QIP application.

  • The first time you login to the e-QIP system after June 24, 2012, you will need to register. You register by clicking the "Register for Username and Password" button on the right-hand side of the screen. You will need to input your Social Security Number (SSN) or request ID if you do not have a SSN, answer your default Golden Questions and then you will create three Challenge Questions. It will then prompt you to create a new username and password. If you have already registered and do not remember your username or the answer to your Challenge Questions, you must contact your sponsoring agency. Note: You can only log into e-QIP when your sponsoring agency grants you access.

  • No, the answers to your Golden Questions and Challenge Questions are not case sensitive. You must type them in exactly as they were entered the first time. If there were any spaces or punctuation in the response, it must be exact.  If you continue to have issues with your Golden or Challenge questions, please contact your sponsoring agency.

  • Providing information that is as complete and accurate as possible will assure that your investigation is completed in an efficient and timely manner. If you are unable to answer a question with precision, provide approximate information, note that you have done so on the questionnaire.  If you are interviewed in person, point out the approximated information on the questionnaire to the Investigator. Estimate the date (month and year) to the best of your ability and check the "EST" checkbox, indicating that this is an estimated date.
