Background Investigations

To obtain a copy of your background investigation conducted by OPM's Federal Investigative Services Division, please mail your request to the following office. If you have any questions, you can call them at 724-794-5612:

Your request must include your hand written signature and all of the following information:

  • Full name
  • Social Security Number
  • Date of birth
  • Place of birth
  • Current home address (a Post Office Box is not acceptable; the records are sent by certified mail and require your signature).

Alternatively, you may fax your hand signed request to telephone number 724-794-4590.

Personnel Records

Current Federal Employees

The Official Personnel Folders (OPFs), including SF 50s, for current Federal employees are stored at the employee's current employing agency. The request for the records must be made directly to that agency's Freedom of Information/Privacy office. The Department of Justice (DOJ) lists the current agency Freedom of Information/Privacy Contacts on the DOJ website.

Former Federal Employees

The Official Personnel Folders (OPF), including SF 50s, for most former Federal employees are stored at the address below. Please mail your request to:

Your request to either the NPRC or the U.S. Postal Service must include your hand written signature and all of the following information:

  • Social Security Number
  • Date of birth
