Learn how to request a mailed copy of your personalized annuity booklet, which we send to all annuitants when they start receiving payment. You can request a copy of the original booklet we sent you or a copy of your current booklet, which will have the most up-to-date information.

Sign in to your online account

Click Documents in the menu and then click Request Booklet

Choose Request Current Booklet or Request Original Booklet
We'll send your booklet to the address we have on file. You can verify or change your mailing address by clicking on Profile in the menu and then clicking on the Communication tab.

You should only fax us documents if an official OPM form or one of our Customer Service Specialists asks you to (the fax number will be provided on the form, or the Customer Service Specialist will provide you one.) Otherwise, you should mail us your documents.
