You should contact Court Order Benefits at 1-888-767-6738. All orders (Termination and Amended) for Child Support, Alimony or Bankruptcy should be mailed into OPM Court Ordered Benefits Branch, P.O. Box 17, Washington D.C. 20044. If you are seeking to obtain benefits as a former spouse for an apportionment, please refer to the Standard Form 3119, Application for Court-Ordered Benefits for Former Spouses for instructions. Please note a court certified copy of court orders and other supporting documentation are required.
