Your disability annuity was previously terminated because your reported earned income met or exceeded 80% of your current rate of basic pay of the position you occupied immediately before your retirement (or your restored to earning capacity limit). If you believe your earned income fell below the 80% restored to earning capacity limit, fax a letter requesting reinstatement of your disability annuity to (202) 606-0022 with an ATTENTION to: Retirement Surveys and Students-Request for DES Restoration. Please also include your completed Tax Return with all associated W-2s for the year(s) in question.
