No. Your agency is not required to list dependents on the IRS Form 1095-C for enrollees that have FEHB coverage. The Form 1095-B, received from your FEHB health insurance carrier should include information about your dependent’s health insurance coverage.

If you have questions about an incorrect form, you should call your FEHB carrier’s contact telephone number provided in Line 18 on the IRS Form 1095-B. OPM and your employing agency will not be able to answer questions about the information on this form. It is the FEHB carrier’s responsibility to make corrections to and answer questions about the IRS Form 1095-B.

Beginning in 2016, under Internal Revenue Code section 6055, all insurance providers must report coverage information to the Internal Revenue Service (IRS). Your FEHB health plan is considered an insurance provider.
The IRS requires the FEHB plan to provide Social Security numbers for the FEHB enrollee and their dependents. Your FEHB plan may already have your Social Security number, but may not have Social Security numbers of your family members. This information is currently being requested to comply with these requirements.

In early 2016, and each following year, you can expect to receive forms from your employer and your health insurance provider. These forms will have information on your health coverage during the previous plan year. You will use this information when you file your individual tax return.

The Form 1095-C provides information from your employing agency about periods of full-time employment and whether you were eligible to enroll in health insurance coverage. The Form 1095-B, issued by your FEHB carrier, shows the periods you were enrolled in minimum essential health insurance coverage.

Yes, this is a legitimate request. Your FEHB plan may contact you as it is required to report you and your family members' Social Security number as part of new Internal Revenue Service requirements that go into effect for plan year 2015.
