How often is a security clearance done and is it automatically initiated when a person's clearance re-investigation date comes due?

Security clearances are granted on an as needed basis when there is a requirement for access to classified information.  Reinvestigation requirements are set by the position’s risk and sensitivity per the Federal Investigative Standards. Concerning reinvestigation timeframes for sensitive positions, or those supporting clearances, reinvestigation timeframes are set at 5 year intervals.  The Office of the Under Secretary of Defense, in response to an investigations backlog issued interim guidance on January 17, 2017, for Department of Defense components and agencies allowing in some instances for Tier 3 periodic reinvestigations to be conducted at 10 year intervals and Tier 5 periodic reinvestigations to be initiated six years after the date of the previous investigation versus 5 years. The Office of the Under Secretary of Defense also provided additional guidance on requirements for Special Access Programs on February 10, 2017. As of February 21, 2018, no additional guidance has been issued.
